What are the hallmarks of a great facelift surgeon?
1️⃣ Phenomenal Results
2️⃣ Safe Practice
3️⃣ Compassionate, Ethical Care
Your face is the most precious reflection of you, and so it behooves you to seek out the best of the best in these three arenas.

First, start by looking at results and many of them: you can evaluate quickly a surgeon’s artistic and technical abilities. This is the most straightforward “filter”.

Second is safety: this is a bit more challenging to ascertain though key questions include:
* What is your bleeding/hematoma rate (Hint: should be very low — mine is 1/250; the national average = 1/25)
* What are the qualifications of my anesthesia team? (Hint: headed up by a Board certified anesthesiologist)
* How is the surgical facility equipped to handle emergencies: airway problems such as sudden collapse or constriction (“laryngospasm”), aspiration, lung swelling (“flash pulmonary edema”), severe allergic reactions?
* Does the anesthesia team have the ability to protect my airway with a tube if or when necessary? If not, then why not? (Hint: airway protection, to prevent all the issues above, is essential — even though risks are small, they’re not zero and potentially life threatening).
* If I need to be admitted overnight to a medical center, how is this done and how long does it take? (Hint: Anything longer than a few minutes should be questioned; in my practice we’re physically inside a major medical center so the time it takes to access the highest level of medical care = zero.)
* Are all staff ACLS certified in emergency care? (Hint: should be yes across the board).
* Will I see you (the surgeon) on within the first week post-op? If not, then why not? (Hint: I do.)
* How many facelift do you do in a day? (Hint: should be only one).

Third, compassion and ethical care: you can get a sense during consultations and how the staff attend to your needs. As the saying goes “the way someone does one thing is the way s/he does everything”. This is more gut instinct but ask directed questions and don’t worry about offending since it’s your face, and defensiveness may be a sign for you to move on.

I want to share with you a deeper expression of “my why”, and what moves me as a plastic surgeon when helping patients experience more of their own natural beauty. So, here’s my take on beauty as a concept, which is such a vital part our humanity.

No one can deny that we’re all drawn to beauty instinctively in all its forms — in music, in nature, in art—because it stirs something deep within us that can’t be captured by words. The same essence that makes a beautiful seascape, sculpture or dance feel whole, balanced and in harmony, is the same essence in a beautiful face. As a surgeon committed to the highest expression of my art, I’m guided by principles of natural timeless artistry—an approach that values restraint, subtlety, and respect for the unique structure of every face.

My goal isn’t to change one’s identify, but to help elevate an inner sense of self. Surgery can never manufacture self esteem. Rather, for those who are grounded in self love, yet quietly held back by a feature that doesn’t feel aligned, aesthetic improvement can be a powerful, liberating step forward. When surgery is done thoughtfully, with care and intention, it should never erase identity or take faces to extremes—rather, it elevates so that a face’s natural beauty shines brighter, and not the work behind it. When my patients feel more deeply connected to their outer appearance, the confidence that emerges is carried into every part of life. This is the essence of “my why”.

💎 There are some pearls of wisdom in my beautiful patient’s story:
1️⃣ She was most concerned about the visible cords in her neck, so-called “platysmal bands”, which were made worse by liposuction that she had done in Omaha. Not only does liposuction, or any superficial fat reduction treatment, make bands worse because de-volumizing is like skeletonizing the neck (never a good thing), but it’s the absolute wrong treatment for bands. Bands are caused by a “bowstringing” effect — like a cord connected from jawbone to collarbone that tightens and bows outwards — as we age this worsens. The only way to correct bands is to disconnect the base of the platysma muscle from the collarbone (a safe step called “platysmotomy”) which allows the platysma muscle to elevate more effectively during platysmaplasty, the lifting step that defines the jawline and neck angle. Not doing one or the other will have a high risk of band recurrence and poor neck lift result.

2️⃣ She said that one of her goals is a “preventative facelift” where she can enjoy a subtle refresh and at an earlier age. As a modern concept, this makes tremendous sense because deep plane facelifts done with high skill and artistry, combined with healthy strong tissues, can lead to subtle refreshes that loose fantastic, last 15-20 years and allow patients to enjoy their results for more of their lives. Gone are the days of waiting until you’re in your 60s.

3️⃣Do your research! As she did, look at not only procedures and doctors, but look at many results and listen to your instincts. Then go deeper and ask about how patients are kept safe — the key in my practice is surgery in the safest possible environment —a hospital-based surgery center— where I work with expert anesthesia and nursing teams in a facility equipped to handle anything. Safety + skill = Priceless!

“Facial Proportion 101”: For patients with disproportionately long foreheads or high hairlines that cause facial imbalance, forehead reduction (hairline lowering) surgery is a powerful tool in facial optimization. Oftentimes this manifests as an overall long face as compared to its width, relative small central features such as eyes and lips, and dominating forehead by its sheer surface area.

Surgically I make a precise “micro zigzag” incision that carefully preserves each thick hair follicle (thin “vellous” or baby hairs don’t contribute to scar camouflage and so are removed) along the natural hairline. I release the scalp attachments and gently stretch it forward, anchoring to the bone surface to offload tension from the incision line. This allows the scar to heal imperceptibly as a thin line tucked under hair follicles that grow through it.

This approach aligns with principles of visual attention studied in the 1960s by Yarbus, who demonstrated through eye-tracking research that human gaze is predominantly drawn to the eyes and mouth. Surgical techniques that enhance or draw attention to this “triangle of focus” can therefore contribute meaningfully to perceived beauty.

Hairline lowering is one such indirect method. In women, it can soften the facial frame and create a more youthful, neotenous aesthetic. In men, when paired with other structural refinements, it can improve proportion without compromising masculine definition. When carefully planned and executed, forehead reduction contributes to overall facial harmony by guiding attention to the expressive features of the face.

Facial fat grafting is the gold standard to restore youthful features — and I use it for 3 specific aesthetic effects:
1️⃣ Volume Restoration
With age, tissue atrophy deflates the soft tissue envelope, causing droop and wrinkles. Fat grafting powerfully revolumizes the lips, under eyes, cheeks, temples and even the forehead, for natural restoration of youthful plumpness.
2️⃣ Contour & Smoothness
Natural ogee curves must blend seamlessly across facial subunits. I use ultra-fine cannulas to gently disperse fat in micro-volumes for smooth results. I can even subtly increase protection, curvature and blend across borders.
3️⃣ Skin Rejuvenation
Fat contains both adipose cells and also stem cells, magical cells that can give rise to any cell type. Although science has yet to decide the exact mechanisms, we know that stem cells have a rejuvenating effect on the surrounding skin through powerful chemical signals.

Patients often ask: “What if the fat doesn’t take?”
Incomplete take usually results from poor tissue quality—or poor technique. Thin, poorly vascularized tissue can’t support fat well. And injecting large volumes instead of layered micrografts reduces success. Be that as it may, in skilled hands fat grafting works very well when used properly and not overdone. Natural-looking results depend on proper bone structure. That’s why I often combine fat grafting with bone-enhancing implants for support and balance, rather than over volumizing to make up for lacking structure.

💯 Two years post-op and what a transformation for this nice gentleman who flew in from the Midwest for surgery. He came to me concerned about a heavy neck, weak chin, and lack of definition in his lower face. He also wanted brighter looking eyes.

I treated him with a deep structural neck lift, deep plane facelift, chin augmentation, and upper blepharoplasty. His case was especially complex: his salivary glands and deep neck muscles — including the digastrics and strap muscles — were significantly enlarged, and his hyoid bone sat unusually low, making the neck angle hard to define.

This was a classic example of volume mismatch — where the deep tissues of the neck don’t fit within a small, recessed jaw. Traditional neck lift with platysma tightening alone wouldn’t have achieved this result, nor would his scars have healed so well. He needed deep volume reduction and skeletal augmentation to optimize results. Now, his jawline is strong, his neck defined, and the results look completely natural. And his eyes are brighter which only complemented his brighter more attractive look.

1. What is a “beautification —or ‘optimization’—facelift”? In my view it enhances a face’s attractiveness towards an “aesthetic ideal,” whereas a rejuvenating facelift restores a face to a prior appearance that has shifted with the passage of time. While a youthful look can be a result of both, my approach to face optimization surgery targets the 4 psychological “beauty cues” to which humans are wired to respond: neoteny, symmetry, sexual dimorphism (features that conjure masculinity or femininity), and supra-averageness (think overall facial harmony). An “optimized” face looks natural yet induces the observer to resonate with beauty cues more intensely than the prior appearance — a subconscious effect. Surgical modification of anatomy to strike the right balance of these cues, and not overdoing one or the other, is the art, while patient goals and anatomy are the drivers, not simply age.

2. How do I do it?
In younger patients and for those who aren’t showing classic signs of age, the indication for an optimization facelift is a bottom-heavy appearing face. While often mistaken for premature aging, the real driver often is inadequately projecting bone structure — here, the skeletal framework can’t support tissues and drooping creates a sagging, pseudo-aging appearance. In this situation, we first need to enhance the underlying framework so that a facelift can be effective and last. On the other hand, a facelift done on an unsupported frame is like hanging a heavy ballgown on a thin hanger — it’s destined to slide off. So my strategy first is to optimize bone structural support by augmenting the midface, and deep plane face and neck lift to reposition tissues on a strong foundation. This can often be done safely in 1 surgery.

There are many other nuances to optimization but the most important is safety. I ensure that every surgical plan is tailored to anatomy and goals, and that each patient is physically and psychologically optimized before embarking on life improving surgery.
