Overview of Undereye Bag Removal in Washington DC

Lower Blepharoplasty, otherwise known as eyelid contouring for the lower eyelid, is a procedure designed primarily to correct the presence of severe undereye bags and dark circles that can make us appear fatigued.

Overtime, the fatty tissue behind the eyes can bulge forward and the tear trough indentation is what causes the appearance of the dark circles. If the darker coloration under the eyes is minimal, it’s recommended to start with a more subtle approach to treatments like injectable fillers. This can be a worthwhile and minimally-invasive solution that temporarily masks the appearance of undereye bags. For more severe dark circles under the eyes, the gold standard is surgical contouring by Dr. Rad, which offers scarless results that visibly remove the appearance of dark circles and the fatigued look that often accompanies it.

Patients of Dr. Rad’s who have undergone lower eyelid contouring are amazed at the subtle, yet transformative results his years of expertise provide.

Undereye Bag Removal

Dr. Rad’s approach is unique for each patient, and it combines sophisticated, state-of-the-art techniques to achieve natural and long lasting results.

Am I a Good Candidate for Undereye Bag Removal?

As previously mentioned above, surgical removal of undereye bags should be reserved for moderate to severe cases of dark circles only. Though dark circles tend to develop as we get older, this procedure is not age restricted. Many people can struggle with bags at an early age due to several genetic factors. As with any surgical procedure, patients for lower blepharoplasty should be of good overall health and have a firm understanding of what the procedure can and cannot do. An ophthalmology evaluation is needed to ensure that there is no evidence of retinal disease or glaucoma.

If you are interested in the procedure but wondering whether you’re a good candidate for lower eyelid contouring, Dr. Rad would gladly sit down with you during your consultation and discuss your aesthetic goals and determine whether the procedure is right for you. He may suggest an alternative such and fillers or other related procedures. In some cases, a combination of treatments like facial fat grafting in order to restore volume to deflated cheeks and under-eye regions.

How Does Dr. Rad Approach Undereye Bag Removal?

Undereye Bag removal with Dr. Rad is achieved by surgically removing the three small fat pads behind the eye that have progressively bulged forward over time. However, the tear trough beneath the undereye makes puffiness appear worse. Like a hill going into a valley, reducing the hill and filling the valley can level the terrain and Dr. Rad employs two approaches to achieve this: (1) transconjunctival lower blepharoplasty plus fat transfer: this approach features a hidden incision inside the eyelid through which protruding fat pockets can be removed safely and effectively. Ideal candidates for this approach are those with good skin tone and elasticity. The other procedure is called (2) subciliary lower blepharoplasty with fat transposition. This is best for patients with puffiness and also wrinkled lower eyelid skin, and involves an incision beneath the eyelid. Dr. Rad carefully separates the skin from the underlying muscle to redrape and smooth it, and then he shifts (transposes) fat pads into the tear trough region. This is one of the more technically challenging methods to perform well. Dr. Rad performs these procedures in an accredited surgery center while the patient is under light anesthesia, delivered by a board-certified anesthesiologist to maximize patient safety and comfort. Surgery involves very little discomfort. Mild bruising and temporary swelling are normal and resolve quickly. While healing and recovery will be different for each patient, return to work or going to social outings about 7-14 days after surgery is common.

Q+A

WHAT NON-SURGICAL OPTIONS DO I HAVE?

Collagen-stimulating treatments such as Laser restore skin elasticity and, when combined with a daily eyelid-focused skincare regimen the need for surgery can be delayed. Injectable filler can be used to fill the tear trough, but often this does not achieve 100% correction and also are temporary.

WHAT IS THE RECOVERY FROM UNDEREYE BAG REMOVAL?

Surgery is outpatient and you’ll experience little discomfort easily controlled on mild pain medicine. You’ll have very little need for strong pain medicine. Mild bruising and temporary swelling are normal and resolve quickly, typically over 3-5 days, depending on the extent of your procedures. Dr. Rad’s detailed pre- and post-operative preparations ensure that you can achieve results quickly and with minimal down time. While healing and recovery will be different for each patient, return to work or going to social outings about 5-10 days after surgery is common.

WHY DO I LOOK TIRED?

Undereye bags make us look tired or older. The problem is two fold: (1) protruding fat pockets cause the lower eyelids to bulge out, and (2) deep tear troughs, the indentation that appears underneath our lower eyelids, makes puffiness appear worse. The combination causes dark circles under the eyes which make us look tired or older.

HOW DOES SURGERY MAKE ME LOOK RESTED?

Surgery must address both puffiness of the eyelids and the indentations below them. Dr. Rad focuses on creating a smooth contour from eyelid to cheek called the “lid-cheek junction”. Smooth contour translates visually to a rested appearance. Dr. Rad tailors his approaches uniquely to each patients anatomy to minimize downtime.

I HAVE WRINKLED EYELID SKIN – CAN THIS BE IMPROVED?

Yes: Dr. Rad’s subciliary lower blepharoplasty with fat transposition approach beautifully addresses wrinkled, crepe-y skin to rejuvenate the eyes and remove 15-20 years off the appearance of the eyes.

WHAT IS DOWNTIME AFTER EYELID BAG REMOVAL?

The transconjunctival approach involves typically 7-10 days of social downtime. The subciliary approach, because it is somewhat more involved, involves 10-14 days of downtime. But keep in mind return to work depends on the nature of your interactions: patients who work from home may feel comfortable with just a few days of downtime, whereas TV personalities may wish to wait an extra few days to a week so they can feel comfortable on camera.

WHEN CAN I RESUME EXERCISE?

Dr. Rad works with each patient to meet their unique recovery needs. Return to full exercise varies with how vigorous activities are. Generally speaking, avoidance of heavy lifting for 3 weeks is advisable, however walking, elliptical and light biking are fine within a few days after surgery.

IS EYELID BAG REMOVAL SURGERY SAFE?

Yes! The techniques Dr. Rad uses are tried-and-true and ones Dr. Rad has used in treating many hundreds of patients successfully! Of course, your safety is the most important factor which is why Dr. Rad chooses to perform surgery in a hospital operating facility with an anesthesiologist, and not in an in-office surgery room with non-physician anesthetists.

CAN OTHER PROCEDURES ENHANCE MY RESULTS?

Yes! Dr. Rad often performs additional procedures which have additional rejuvenating effects on the lower eyelids: facelift, brow lift, upper blepharoplasty, fat transfer, canthopexy can all be employed safely and simultaneously. Consulting with Dr. Rad is the

WHERE ARE THE SCARS?

The transconjunctival approach involves completely hidden incisions whereas the subciliary approach involves scars just under the eyelash line. While technically visible these scars usually heal beautifully and imperceptibly.

Dr. Rad will spend time carefully listening to your concerns, asking clarifying questions and then masterfully guiding you, step by step, to understand how your unique anatomy can be improved to convey a rested appearance. Dr. Rad’s goal is to determine the most comprehensive approach for long lasting, natural results while minimizing downtime. Dr. Rad first needs to understand your specific concerns and you should prepare your thoughts prior to your consultation. Think of words that convey how you feel about your appearance. For example, Washington DC patients often say they look fatigued, tired or lackluster because of undereye bags or dark undereye circles, or they look sad, upset or disinterested such as from heavy eyebrows, excess skin above the eyes or downturned corners of the mouth. Patients often relate concerns of looking older than how they feel and this can be caused by jowls, nasolabial folds, loose neck skin, lines and wrinkles. Try to put into words how your appearance bothers you, and the priority you give to each area. Dr. Rad will then present to you his expert opinion about which procedures would be best to restore your anatomy to create a rested and refreshed appearance. And since surgery is intricate Dr. Rad will guide you through a stepwise approach of aesthetic analysis and he’ll deliver his recommendations in a clear and honest manner. Dr. Rad will present all options available in modern day surgery, both non-surgical and surgical.

Dr. Rad and Board certified dermatologist Dr. Sherber often collaborate to optimize patients’ results and to minimize invasiveness. As such Dr. Rad’s modern approach to surgery harnesses the power of both dermatology and plastic surgery as both specialities address different aesthetic challenges. For example, plastic surgery is effective to lift, tighten and volumize loose, atrophied tissues, whereas dermatologic treatments such as laser, radiofrequency tightening and skincare are best to improve skin texture, pigmentation and tone. Dr. Rad can address any concerns regarding skin quality and how to obtain and maintain optimal results.

Dr. Rad will review with you photos of his patients’ results (many of which you’ll not find online due to privacy concerns) to highlight his aesthetic approaches and skill. He will take your photographs, which remain confidential, to assist with the planning of your surgery. For your safety Dr. Rad performs surgery in a hospital-based operating room, considered to be the safest environment, rather than in the office. Anesthesia is delivered by a board certified anesthesiologist (MD) and you’ll be under light anesthesia which is important for your safety and comfort.

You will receive a customized quote for the procedures in which you’re interested. Once approved we may then move forward with pre-operative preparations and you will meet with Dr. Rad again at your final in person consultation prior to surgery.
