Deep plane facelift and deep structural neck lift are the foundation of my approach to create naturally beautiful, long-lasting refreshed results.✨ What sets this apart is that it’s not just about lifting skin—it’s about reshaping the anatomy beneath.

To truly optimize the face and neck, we have to understand the five anatomical layers and how they should modified:
1. Skin (layer 1) — a tensionless redrape is key.
2. Subcutaneous fat (layer 2) — preserving is key for plush smoothness form one subunit to the next.
3. Musculoaponeurotic layer (SMAS/platysma) (layer 3) — this is key layer for lifting.
4. Deep spaces and ligaments (layer 4) — the “deep plane” where total ligament release is key to mobilize layer 3.
5. Deep fascia (layer 5) — protects facial nerves and should not be violated.

Each layer plays a role in aging and must be addressed strategically for natural, lasting results. That’s why deep plane techniques, which work within the musculoaponeurotic and deep layers, are so powerful—they allow true repositioning, not just surface lifting.

When it comes to the neck, I go deep:
1. Reducing (not removing) the submandibular glands for a smoother jaw contour and defined submandibular (under jawbone) hollow.
2. Reduction of deep subplatysmal fat and anterior strap muscles (digastric) for a sleek, supported neck
3. Lateral platysmaplasty (neck@muscle tightening) and inferior platysmal myotomy with central corset platysmaplasty to correct platysmal bands.

Wow, she’s glowing after Total RAD ReFresh! 💫 What stands out in her transformation is her natural harmony from top to bottom where all facial subunits balance and flow—especially with brightened eyes, corrected neck and jawline definition, and improved facial proportions. Deep plane facelift and deep structural neck lift are the foundation of my approach to creating naturally beautiful, long-lasting facial rejuvenation and optimization.

These diagrams highlight my personal apprach to deep plane face and neck lifting. In the first 2 images, there are 2 tissue layers, or “planes,” of dissection: the yellow highlights the region of skin and subcutaneous fat (layers 1 & 2) lifted from the underlying muscle/fascia (ie SMAS, layer 3) from the ear to the blue line.

The blue layer is the deep plane (layer 4), a gliding layer that allows movement of layer 3 over layer 5 (deep fascia) — it’s here in layer 4 where we dissect and cut the ligaments that anchor all tissue layers to the bone (“osteocutaneous”), crucial to mobilize the cheek fat pads that have descended into the nasolabial folds and marionette lines (“labiomandibular folds”). Once mobilized, all layers and cheek fat lift up and back, and the excess skin is trimmed in a tension free closure.

The red region represents the deep structural necklift which I show in images 3 and 4: here the saliva gland, suspended by the digastric muscle sling, is oversized and reduced, with the hyoid bone pulled back by tightening the platysma. The sub-platysma fat and strap muscles are reduced and shaped to optimize and redefine the cervicomental angle for a more elegant, youthful neckline.

Unlike superficial techniques that only tighten skin, the deep plane method releases and repositions the deeper fascia layers—restoring volume, softening folds, and recontouring the jawline without the tension or ‘pulled’ look. Every face is unique. That’s why each procedure is tailored using a precise, artistic eye and a deep understanding of facial anatomy. The goal isn’t just to reverse time—it’s to optimize your features so you still look like you, just refreshed, refined, and radiant, and the key is anatomy. 🔑
