🩶 My ultimate goal for patients is their happiness that comes from the confidence of looking their most beautiful, “optimized” self. My lovely patient was concerned for early signs of aging, in particular loose hanging neck, jowls, irregular laxity of her midface, eyes looking fatigued, and just an overall deflated appearance. In consulting with her it became apparent that her optimized results would come through by both rejuvenation and optimization strategies. 🎯

The difference between the two is that rejuvenation comes about from restoring a prior youthful appearance (i.e. “rewinding the clock”) through deep plane face/neck lift, deep neck sculpting and lower blepharoplasty whereas optimization is creating something “new,” that is, a naturally enhanced and alluring appearance never before had. I create this primarily in the eyes through endoscopic brow lift and canthoplasty “fox eye” lift. Finally, I use fat grafting as a way to smooth surface contours with subtle volumizing, like “snowflakes on a field” rather than large volumes in hollow areas. I treated her forehead, temples, midface, under eyes, upper and lower lips, and I try to create a natural softness that feminizes the face which I think she has.

That patients put their trust in me is a responsibility I take very seriously and for this reason patient safety is my top priority. This being true I take care of patients in a hospital-based surgery facility where we have many board-certified anesthesia teams and highly skilled nursing teams and they can handle anything. Even though the risk of anesthesia-related complications is very low for young healthy patients, they’re not zero and so I don’t even take that chance. Having this peace of mind for myself and my patient that we’re operating in the safest environment and achieving best results.💯

Deep neck sculpting (DNS) is the most modern approach to aesthetic neck lift. DNS refers to reducing, shaping and tightening l 4 under-platysma (ie “deep”) tissue structures:
1️⃣ subplatysmal fat
2️⃣ anterior “strap” muscles (digastric, omohyoid, mylohyoid)
3️⃣ hyoid fascia
4️⃣ submandibular saliva glands

The glands (#4️⃣) are most important because, as we age, they grow larger and drop creating fullness in the neck. Patients often mistake a fill neck as extra subcutaneous fat and injectors offer Kybella or liposuction, the least effective treatments. In the setting of large deep structure, traditional neck lift only with skin and muscle tightening is limited or fails because the glands are pushed downwards by the “floor of mouth” structures. It’s like trying to lift up bowling balls suspended in a hammock just by pulling on the ends.

So the glands must be reduced, like offloading the platysma “hammock,” allowing the neck and jawline to be lifted and tightened. Not doing so can lead to minimal neck and jawline improvement, widened scars and pixie ears deformity, problems I see very often in patients who’ve had prior neck lift without DNS. But with deep sculpting we’re ensuring volumetric proportion and optimal aesthetic results. 💯

🎯 My sole aim of surgery is to have a happy patient in the end, and I’m thrilled for this lovely gentleman from out of state, a fellow physician and cosmetic dermatologist, whose beaming smile says it all. Bothered by an aged appearance that didn’t match his energy, especially in the neck and eyes, his goals related both to rejuvenation, to reverse the signs of entering his 8th decade of life, as well as “face optimization” to correct deficient bone structure: his set-back eye socket bone (called “negative vector orbital rim”) exacerbated under eye bags and deep hollows that soft tissue-only procedures would have been limited to correct.

In consultation we decided that a 2 staged approach would be optimal for results and safety, first addressing the lower half with deep plane face and neck lift, deep neck sculpting (gland reduction), facial fat transfer and upper blepharoplasty. Approximately one year later we addressed his under eyes with subciliary (under eyelash incisions) lower blepharoplasty, fat transposition, lateral orbicularis suspension and canthopexy, orbital rim porous polyethylene (PP) implants and endoscopic lateral brow lift. Restoration of his youthful appearance was enhanced by correcting his negative vector under eye anatomy for which the only effective method is skeletal implants, and for many reasons PP implants are the gold standard.

Now his appearance matches his youthful energy and he has a spring in his step from newfound confidence in looking his best. Frequently he tells me of all the compliments he receives from his patients who believe he’s aging backwards, many often guessing that he’s in his 40s when in fact he will turn 70 in a few days. Rewinding the clock 20-30 years often requires these more comprehensive procedures and, albeit more involved, is a testament to the power of total facial optimization that can lead to transformative yet natural appearing and permanent results. Face optimization is about subtle shaping of many contours in multiple vectors throughout the face. My patient came from New York City for a complete yet natural transformation to improve her features in a harmonious way. She lamented that her face had a heaviness, sadness and proportions that gave her a face an unfeminine, bottom heavy quality. Not only did she want to look refreshed but also naturally optimized.

Aesthetically, my guiding principle is that all procedures must serve to enhance the eyes and lips, the central triangle on which humans are wired subconsciously to focus when assessing a face. Low and asymmetrical brow and eyelid shape framed her eyes in a downward configuration. As our brains are wired to interpret this as sadness or fatigue, by reversing the shape, the emotive nature gives way to positivity and allure. Her lower eyelids had a fullness above the orbital rims (eye socket bone) that accentuated the tear troughs, creating dark under circles and an aged appearance. Midfacial volume and contour were subtly lacking high cheek convexity, and she felt that her prominent jaw framed her lower face in a bottom heavy way. Finally, she wanted tastefully enhanced lip volume and refined nasal shape.

I often use techniques that enhance them “directly” and “indirectly.” By “directly,” I mean shaping the eyelids and lips themselves: for example, upper lip lift plumps rolls the pink part outward, giving the appearance of volume. And endoscopic brow lift and canthoplasty “fox eye” lift has a powerful effect on the eyes. Lower blepharoplasty effaces the tear troughs and defines the infratarsal crease. By “indirectly”, I mean reducing surrounding facial structures to make the eyes and lips look larger — for example, reducing the jaw and nose by V-line jaw slimming and rhinoplasty has a proportionality effect that does this.

For safety we planned a two staged approach. Together our goal wasn’t a dramatic transformation but rather subtle enhancement, and the guiding principles of aesthetics that I’ve cultivated ensure naturalness, harmony and optimized beauty.

“Face Optimization” is my personal quest in aesthetics with the sole aim of giving my patients the most harmonious, beautiful and natural appearance as seen through their eyes, looking as though no surgery was ever done. 💯

This is bespoke surgery, not cookie cutter, and I have 3 guiding principles with synergy as the common thread between them:
1. Create subtle enhancement in many areas throughout the face. This means multiple procedures that work in synchrony together.
2. Treat suboptimal bone structure and soft tissue volumes appropriately. With optimal bone structure, aesthetic results are more natural appearing and last because soft tissues are well supported allowing patients to age gracefully.
3. Modify facial proportions so that the eyes and lips are brought to the foreground, and all else fades to the background. The central facial triangle is the art, all else is the frame.

I rely on PP implants, skeletal contouring, deep plane lifts, fat transposition techniques and conservative fat transfer as these techniques give my patients optimized results with longevity and minimal scarring. Overall the goal of facial optimization is feminine beauty or masculine attractiveness without stigmata of surgery. And my ultimate target is for my patients to feel the most confident and happy knowing they look their best. 🎯

🤍I took care of this lovely young lady when she had a double mastectomy to prevent breast cancer and I reconstructed her breasts with bilateral DIEP flaps, a major microsurgery. I then had another wonderful opportunity to take care of her again to boost her confidence in her facial appearance as she wanted a more sculpted neck, chin and lower face.

Endoscopic “ponytail” lower face/neck lift is scarless in front of the ears with small incisions tucked behind them and under the chin. Perfect for patients with no excess skin but large deep neck structures like she had, (saliva glands, under platysma fat and strap muscles), that bulge outside the jawbone. Through small incisions in 3 places I separate the skin from muscle, reduce the glands and tighten muscle without evidence.

She looks naturally sculpted now and the beauty is that this result is permanent and last forever. 💯

👃 Long term results show stability over time and this is especially important for rhinoplasty. My patient disliked the hump on her bridge, droopy and asymmetrical tip and wide upper nose. She wanted a straight bridge with better symmetry and without dramatic change. Her goal was to retain the natural character of her face and not a reducing to a tiny “button nose”. This was an important goal considering that her face has strong features and a cookie cutter Barbie nose would draw attention for the wrong reasons.

I treated her with a closed approach, slight dorsal hump reduction, septoplasty, tip shaping, cartilage graft to radix, and osteotomies (nasal bone narrowing). The one week result shows the initial swelling that is typical, but this resolves quickly and at 10-12 days patients look very presentable and can return to normal activities.
