Thrilled for my patient 6 months who wanted to brighten her eyes subtly, optimize her face and neck contours and to correct disharmony of her nose.

Her Rad ReFresh+FaceOps combination of:
* Deep plane facelift + deep structural necklift
* Endoscopic deep plane browlift, subtle canthoplasty
* Microfat grafting
* Lower blepharoplasty fat transposition
* Closed rhinoplasty

The goal was subtle optimization throughout her face to bring more focus to her beautiful eyes. Before surgery she felt that her brows and eye shape were suboptimal, perioral mounds caused harsh shadowing around her mouth, hollowing through temples and under eyes accentuated fatigue, and laxity and fullness of her neck belied her young age. My signature deep plane lift repositioned the full contiguous layer: platysma, SMAS and temporoparietal fascia — the key to beautiful naturalness.

Love this soft, subtle upper face refres

Love this soft, subtle upper face refresh for my patient who wanted to brighten her eyes and complexion, and also correct disharmony of her nose.

The nuance is in the details:
Appreciate not only how her eyes are brighter, but her entire complexion is smoother by virtue of the fat grafting with endoscopic forehead lift. I also created soft convex “Ogee” (ie S-shaped) curves which you can see in her forehead as it flows into her nasal bridge, to her temples laterally and around her eyes and midface.

Her nose was particularly challenging due to heavy scar burden from her prior open rhinoplasty (done elsewhere). These situations are particularly unpredictable given that he nasal cartilages were encased in scar and so a minimally invasive closed approach is most effective. In her case, hiding internal nostril incisions gave access, I removed the above-tip folders and then created tip projection with grafts. Minimally invasive optimized results and minimized instability of the tip complex.

Overall I’m thrilled that she shines more brightly and can enjoy this one year result for many years to come.

Here’s a fun one: “ChatGPT: what do you think of this result?” Glad to get a 9.7 from a tough critic! This result for an absolutely lovely patient who lost 100+ lb is a perfect example of combined deep structural work in both the face and neck to restore definition and youthful contour through complete tension free soft tissue redraping.

Key elements to my approach included:
• top to bottom lift of the contiguous deep plane — platysma, SMAS & temporoparietal fascia (TPF) for durable vertical elevation of the jawline, midface and brows.
• Subplatysmal neck sculpting including removal of deep fat, digastric muscle and submandibular gland reduction for a sharp cervicomental angle and crisp jawline.
• completely tension-free redraping of skin for perfect scar healing.
• Preservation of native landmarks, including earlobe and tragus integrity
• microfat grafting for optimal skin texture and complexion enhancement
• chin augmentation for subtle structural rebalancing.

One word…stunning ✨ She felt self-conscious about facial features that didn’t reflect the femininity that she felt, specifically a beautifully yet oversized and strong jaw structure that gave somewhat masculine facial proportions, a dorsal nasal hump and slightly bulbous, dropped tip that drew attention, slightly heavy and low set brows that she compensated with “cat eye flick” mascara, and subtle volume deficiency in her forehead, temples, under eyes and lips.

Meticulous planning through my bespoke signature “FaceOps” (facial optimization) approach, my goal was to fully optimize her facial proportions, enhance her eyes in subtle yet transformative ways. Key to this plan were:
• V-line jaw and chin reduction: I removed ~8-12 mm of vertical height from the back of her jawbone (“ramus”) tapering to the front, and shaved her chin to narrow its width. This reduced the height of her lower face while still maintaining Angelina Jolie-esque jaw structure, bringing femininity to her face while still preserving the character.  • Deep plane platysmaplasty neck lift: this was a “preemptive strike” to maintain sharply clean jaw definition and a gonial angle, as well as neck contour in the context of jaw reduction which can lead to blunting of these lines.  • Endoscopic brow lift & canthal release/lift: I gently elevated her outer brow and eye corners bring more allure to her gaze. • Microfat grafting: subtly perfected her facial contours and added youthful volume. She’s very athletic and slender and so microfat dispersed throughout forehead, temples, under eyes, lips and midface enhanced the feminine softness of her beautiful face.

Now, just 3 months post-op and getting better with time, she’s enjoying a profound boost in confidence. She told me that she has even caught the attention of top modeling agencies for her striking beauty. 🔥

I enjoyed taking care of this lovely woman who flew in from the Midwest ✈️ for a total RAD ReFresh and a touch of my signature face optimization techniques. She’s 9 months out from deep plane face & neck lift, deep neck sculpting, endoscopic brow and canthal lift, microfat grafting and closed rhinoplasty. I love her optimized facial harmony, naturally feminine features where everything just fits beautifully. Key technical and artistic points include:

• Subtly reduced submandibular glands, full release of midface osteocutaneous ligaments which de-tensioned the platysma and SMAS allowing them to elevate more significantly— this is a key concept I discuss with patients where the SMAS-platysma is one contiguous sheet that glides over the deep plane (layer 4). Release of the ligamentous attachment of both layers is key to a maximally lifted result.  • Her eyelid tilt and brows are shaped to frame her eyes in a way that brings out their alluring quality.  • Upper orbits (eye socket hollows) are naturally fuller by microfat grafting which also softened her entire upper face from forehead to temples and under eyes, and lips are more ample with better symmetry.

Sometimes a simple post is best — I’m excited for my lovely patient who is just a little over 2 weeks post-combination Rad ReFresh (facial rejuvenation) + Rad FaceOps (face optimization).

Here’s the breakdown:
* The ReFresh component was through my signature extended deep plane face and neck lift, deep structural neck sculpting (i.e. gland reduction), endoscopic (deep plane) brow lift and microfat grafting to restore gentle volume. This is my most powerful method to restore back natural youthfulness in a comprehensive way.
* The FaceOps component focused on her eyes: she lamented downward angling canthal tilt and drooped lower eyelid with tear trough hollowing. I combined my signature “orbital bone anchor” canthal suspension to reorient her eyes gently upwards along with gentle orbital rim augmentation using small HDPPE implants which supported her lower eyelids, and lower blepharoplasty to blend the lid-cheek junction. Note her lifted lower lid position which now slightly overlaps the iris.

Overall she has a natural heart shape with beautified central facial features. At only 2+ weeks postop I love how she’s looking absolutely stunning albeit with swelling and residual bruising, but ever more improvements to look forward to. (NB: She flew in from out of town so I just have an early selfie to show, but looking forward to sharing her formal in-studio photos when she’s able to fly back to DC.)

I’m so happy for my patient who is truly beautiful inside and out.
