Surgical Frown Line Reduction in Washington DC


Surgical frown line reduction is a surgical procedure designed to soften and smooth frown (“number 11”) lines by surgically weakening the frown (“Corrugator supercilii”) muscles. It offers patients a “permanent Botox effect” and it is comfortable to undergo awake with injected numbing medicine.

Dr. Rad often performs this with brow lift and upper blepharoplasty to give patients a bright, warm and positive appearance to the eyes, forehead and brows. Surgery is outpatient and recovery is just a few days with minimal discomfort.

As an in-office procedure the cost of frown line reduction is $4,500. Botox patients often ask whether they will save on Botox in the long run by having this procedure: if you undergo Botox treatment of your frown lines every 3 months, then your surgical frown line reduction will be paid off in about 2-2.5 years (assuming 25 units/treatment), and if you’d otherwise continue Botox treatment for the next 25 years you save $40,000. While total cost increases when combined with other procedures, surgery is cost-effective when multiple procedures are combined. The eyebrow frames the eye and, if it droops, can give a fatigued appearance. Dr. Rad uses 3 techniques for mild, moderate or significant brow elevation, and he tailors the surgery to each patient’s needs.

Surgical Frown Line Reduction Q+A

Surgical frown line reduction should be performed only by a Board certified plastic surgeon. Dr. Rad first assesses your inner brow anatomy. While you are standing upright with your eyebrows relaxed Dr. Rad makes precise measurements in order to assess eyebrow and facial asymmetry at baseline. If planning an upper blepharoplasty at the same time, Dr. Rad marks the skin area to be removed. As a solo procedure Dr. Rad otherwise precisely marks the incision to fold within the upper eyelid crease. Through the incision, Dr. Rad dissects beneath the eyelid muscle (called “Orbicularis oculi”) upward and inward and identifies the Corrugator supercilii muscle (i.e. the frown muscle). Dr. Rad then removes a segment of the muscle which weakens it, and replaces the removed tissue with fat: this disrupts the function of the muscle while maintaining normal surface contour. Dr. Rad asks you to frown in order to check for symmetry. Dr. Rad then closes the incision precisely with fine suture.

Surgical frown line reduction is very safe. The most common complications that can occur are slight asymmetry in muscle activity, and contour depression. Dr. Rad is extremely meticulous in surgery and the chance of these problems occurring is extremely low. Such problems are correctable and patients who have had suboptimal results from other surgeons often seek Dr. Rad’s experience for corrective work.
